High school sports eligibility
Four forms are required for athletic participation. Every student athlete will be required to fill out and sign an Inherent Risk form for their specific sport, a Registration Form, Emergency Card, and the Concussion Awareness Form.Additionaly, a Current Sports Physical MUST be on file with the office. Physicals are good for two years, and incoming freshmen will need a current physical for high school sport eligibility.Click on the link to print a physical form to bring to your health care provider.Beginning with the 2019-2020 school year, Ilwaco High School is excited to announce that we are now offering online registration for all sports programs and activities through Family ID!Family ID is a secure registration platform that provides you with an easy, user-friendly way, to register for our programs and help us to be more administratively efficient and enviornmentally responsible. When you register your student through Family ID, the system keeps track of your information in your Family ID profile. You enter your information only once for each family memeber for multiple sports and/or programs.
IN ORDER TO PARTICIPATE:
- Families must create an account on FamilyID and register your student/s.
- Athletes MUST have a current physical uploaded to your FamilyID account prior to participation.
- Athletes MUST have valid insurance information in your FamilyID registration prior to participation. This includes insurance carrier and ID number.
- Participants in athletics, and activities MUST have a 2019-20 ASB card. Cost is $30. Payment cannot be made online—cash or check only—and can be paid in the school office.
